OLCC County Application/Renewal Form
$100 New application
$75 Change in ownership, location, or privilege
$35 Annual renewal
Fill out the top part of the COUNTY FORM. Send or
bring it to the Health Department with your payment.
Mail to: Tillamook County Environmental Health, PO Box 489, Tillamook, OR 97141
Bring to: Tillamook County Health Department, 801 Pacific Ave, Tillamook (blue entrance)
Fax to: Attn Allison, 503-842-3983; then Call 503-842-3943
to make a payment via credit card.
If you have sent in payment but not the county form, we still need to receive this form before we can process your payment.
Send or bring a copy of the STATE OLCC FORM as well. The Commissioners no longer have to sign the form for renewals, but we still need a copy for our records. Your application WILL NOT BE PROCESSED without this copy.
Your fee and application will be sent to the County Commissioners to be processed. The information will be advertised in the Headlight Herald. The week following the advertising, it will go before the Commissioners at their Board meeting for approval.
This process takes TWO TO FOUR WEEKS, as the application must be routed through a number of departments, and the paper only publishes once a week. Please be sure to give yourself plenty of time for your application to be processed.
