Birth Certificate ordering instructions
Certified birth records can be obtained for:
- All persons born in Tillamook County within the past six months.
- For births registered outside Tillamook County or prior to the past six months, complete the Oregon Health Authority Birth Record Order Form
How much does a birth certificate cost?:
- $20 each or $15 each additional (cash, check, credit card). Make checks payable to: Tillamook County Health Department.
Information you need to provide us to search for a birth record:
- Full name of child (First, Middle, Last)
- Birth date of child
- Sex of child
- Place of birth
- Mother's full maiden name
- Father's full name (if it appears on the record)
- Your relationship to the child (self, parent, or explain)
- Reason for needing record
- Daytime phone number
- Email address
- Name of person ordering record
- Your address, city, state, zip
- Signature of person ordering record
- Identification required (see application for acceptable proofs)
Copies may also be obtained through the Tillamook County
Health Department, Monday through Friday, 8:00 am to 5:00 pm.

Death Certificate ordering instructions
Certified death records can be obtained for:
- All persons who died in Tillamook County within the past six months.
- For records of deaths outside Tillamook County or which occurred more than six months ago, complete the Oregon Health Authority Order Death Record Form
Information you need to provide us to search for a death record:
- Full name of deceased (First, Middle, Last)
- Date of death
- Place of death
- Spouse of decedent
- Your relationship to line #1
- Reason for needing record
- Daytime phone number
- Email address
- Name of person ordering record
- Your address, city, state, zip
- Signature of person ordering record
- Identification required (see application for acceptable proofs)

The purpose of the Medical Examiner's Office is to bring
trained medical evaluation to the investigation of deaths that are of concern to
the public health, safety, and welfare of the community. Accurate investigation
and determination of cause and manner of death are essential to our society.
All requests to obtain a medical examiner, autopsy, or
