Birth Certificate ordering instructions

Certified birth records can be obtained for:

• All persons born in Tillamook County within the past six months.
• For births registered prior to past six months, please contact the State Center for Health Statistics at 971-673-1190 or visit their website at: www.healthoregon.org/chs.

How much does a birth certificate cost?:

• $20 each or $15 each additional (cash, check, credit card). Make checks payable to: Tillamook County Health Department.

Information you need to provide us to search for a birth record:

Listed in order of importance, provide as much of the following as possible. The first five are required.

1. Name of child (First, Middle, Last)
2. Birth date of child
3. Place of birth
4. Mother's maiden name
5. Father's full name (if it appears on the record)
6. Your relationship to the person (self, parent, or explain)

Copies may also be obtained through the Tillamook County Health Department, Monday through Friday, 8:00 am to 5:00 pm.

Death Certificates

Death Certificate ordering instructions

Certified death records can be obtained for:

• All persons who died in Tillamook County within the past six months.
• For records of deaths outside Tillamook County, contact the State Center for Health Statistics at 971-673-1190, or visit their website at: www.healthoregon.org/chs.

Information you need to provide us to search for a death record:

1. Full name of deceased (First, Middle, Last)
2. Spouse of decedent
3. Date of death
4. Place of death
5. Name of person ordering record
6. Your relationship to line #1

How much does a death certificate cost?:

The purpose of the Medical Examiner's Office is to bring trained medical evaluation to the investigation of deaths that are of concern to the public health, safety, and welfare of the community. Accurate investigation and determination of cause and manner of death are essential to our society.
