9 AM to 4:00 PM Tuesday through Thursday, BY APPOINTMENT ONLY! - * THERE WILL BE NO APPOINTMENTS DURING CRUCIAL ELECTION TIMELINES

1. CALL FOR AN APPOINTMENT: (503) 842-3402
2. COMPLETE A DS-11 APPLICATION FORM. These can be obtained from our office or online CLICK HERE .
3. PROOF OF CITIZENSHIP – A CERTIFIED COPY OF BIRTH CERTIFICATE issued by Department of Vital Statistics in the state the applicant was born, a previously issued passport, or a Certificate of Naturalization. Children under 16 must provide a Birth Certificate. Click HERE for Oregon Birth Certificates. Go to www.vitalchek.com for all states.
4. PASSPORT PHOTO - taken within the last six months. Photo size 2” x 2”, taken against a plain white or off-white background. Click here for information on Digital Photographs. Passport Photo services are available in our office for $15.00.
5. CURRENT IDENTIFICATION – Such as a Driver’s License (not an ID card) or other acceptable I.D. Refer to reverse side of passport application. Children under 16 have Special Requirements.
